OUR PROCESS

How we help

1 / Initial Consultation

Our practitioners will have an initial consultation with you and your loved one to ensure we can support you through your therapy journey. The initial consultation will focus on gathering background information, assessment of prior supports and current supports in place. It is also a good opportunity for us to get to know you.

2 / Functional Assessment

A functional assessment will be undertaken by the practitioner. The practitioner assesses every aspect of your life to build a deep knowledge of who you are, your support needs and the function of the behaviour that is being used. We keep a clear and constant line of communication with you whilst we gather further information and develop a risk assessment. At 2inlcude, we will identify your strengths, your motivation and what is important in your life.

3 / Positive Behaviour Support Plan

Our practitioners develop goals and plan strategies by creating a person-centred, evidence-informed positive behaviour support plan. We base your support plan on a shared understanding by identifying the function of the behaviour, what you are trying to communicate and how to meet those needs. Our comprehensive plans will be proactive, reflect your individual needs, improve your quality of life and support your progress towards positive change.

4 / Implementation

When implementing your behaviour support plan, our practitioners will provide tailored support and training so the plan is implemented effectively. At 2include, we believe the best way to have an effective support plan is to be in partnership with you, your support network and other important people in your life.

5 / Monitoring and Evaluation

This ensures ethical and accountable practice that meets your needs. Effective monitoring and evaluation include renewal training, data collection and evaluation between the effectiveness of a behaviour support plan and the effectiveness of its implementation. A behaviour support plan is a live document where the positive results are incorporated, reviewed and updated on an ongoing basis.
